PARENT FORMS AND INFORMATION

CHANGE OF ADDRESS, PHONE, EMAIL ADDRESS, MEDICAL OR CUSTODY

To update your child's information, please click this link.  Please contact your building secretary if you have issues logging into FinalForms. You may enter in a change of address, phone number, email address, medical or custody information. 

Families who are doubling up with another Stow or Munroe Falls resident may use the Sworn Statement of Residency as a proof of residency coupled with the resident's utility bill, lease or home deed. The Sworn Statement of Residency form can be printed here.

 

STUDENT WITHDRAWAL

Parents who wish to withdraw their child(ren) from the Stow-Munroe Falls City Schools may contact the school office of their attending building to withdraw their student(s). Parents may fill out the Student Withdrawal Form in advance and bring along to expedite the process.

BEFORE/AFTER SCHOOL CARE

The Riverfront Family YMCA provides before and after-school child care at the following Stow-Munroe Falls schools:

  • Echo Hills
  • Fishcreek
  • Highland
  • Riverview
  • Indian Trail
  • Woodland
  • Lakeview

Before care begins at 7:00 a.m. and pick-up is by 6:00 p.m. The program is located in the school gymnasium.

Weekly fees for the 2022-2023 school year are:

  • YMCA Program Member: Before Care $65 per week
  • YMCA Member: Before Care $57 per week  
  • YMCA Program Member: After Care $75 per week
  • YMCA Member: After Care  $65 per week
  • YMCA Program Member: Before and After Care $100 per week
  • YMCA  Member: Before and After Care  $90 per week
  • YMCA Program Member: Registration Fee (non-refundable and non-transferable) $40
  • YMCA Member:  Registration Fee (non-refundable and non-transferable) $40
